    International Diabetes :  Doctor, it seems the basic pathology changes of diabetes are the impairment and inflammation of vascular endothelium.  Do those pathological considerations of inflammation assist you in interpreting the findings of your studies?

    Dr Grant :  Well there is very good evidence now that the thrombotic and inflammatory pathways are actually very inter related with each other and form one pathways so there are both pathways that are cellular and circulatory components and they seem to interact across all those different components so macrophages interact platelets components of the compliment pathway interact with thrombosis so it seems that this inflammatory process which involves both systems is actually promoting vascular damage in the wrong place.  It is designed, it is there to protect the body against infection and blood loss and in fact it is causes thrombosis in a distant site.

    International Diabetes :  Switching over to drug therapy, can you see the possibility of preventing thrombosis with drugs which have been proven to be vascular protective?


《国际糖尿病》:对于药物治疗,您认为有可能会利用已经被证明具有保护血管作用的药物来预防血栓形成吗?

    Dr Grant :  I think that the problem we have with the management of thrombosis is that at the current moment we have therapeutic particular proteins in the thrombotic cascade and I think that the complexity of the pathways and the way they interact with each other suggests that actually what we need to do is look for drugs that influence the basic mechanism that cause the activation of both these systems and you would expect, I mean I think that inflammation and thrombosis being interrelated pathways are both involved in the etiology of diabetes and cardiovascular disease and contributes to the development of complication as the disease progresses so you would expect that if you could understand the mechanisms that regulate both those processes you get drugs that ameliorate both conditions.

Grant教授:我认为对于血栓治疗方面我们存在的问题是,目前我们拥有参与血栓形成级联反应的治疗性特殊蛋白,反应途径的复杂性以及它们相互影响的方式提示我们,实际上我们需要做的就是寻找一些能够影响两个系统活化基本机制的药物。我的意思是说我认为炎症反应和血栓形成作为相互联系的两条途径既参与糖尿病和心血管疾病的形成又促进了作为疾病进程的并发症的发展。所以如果你能理解调节它们进程的机制,希望你能找到一些药物来改善这两种状况。
